Change of name, address, phone, SSN 

Complete this form if you have a change in your name (e.g., recently married),  address, phone, or SSN (e.g., if original information provided was incorrect).  Be sure to sign the form, or it will not be processed.

 

Reissue a benefit check

Complete this form to request a benefit check to be reissued (e.g., check was lost, stale dated, damaged).  CAA does not consider a check to be lost until 6 weeks after the check was issued.
